Abstract
Aim
To describe a novel technique of real-time microscope integrated Optical Coherence Tomography (MiOCT) guided stromal fluid drainage in Recalcitrant Corneal Hydrops (RCH)
Method
11 eyes of keratoconus with RCH (not responding to standard therapy beyond 4 weeks) underwent this procedure. The surgical technique included MiOCT-guided localization of the stromal fluid clefts followed by multiple venting incisions into the stromal fluid clefts with MVR with simultaneous air tamponade in anterior chamber.
Results
10 patients (11 eyes) underwent this procedure. The median duration of hydrops before undergoing the procedure was 21± 8.8 days. The median corneal thickness on presentation was 1625± 390 microns. The median resolution time was 17.5± 8.4 days. None of the cases required a repeat procedure.
Conclusion
MiOCT guided drainage of stromal fluid pockets is a safe and effective technique for early resolution of corneal edema in cases of Recalcitrant Corneal Hydrops.
