Abstract

STUDY DESIGN
Prospective Interventional study

PURPOSE
To report the surgical outcome of two rare cases of Helveston Syndrome-the triad exotropia of ‘Dissociated vertical deviation,Superior oblique(SO) overaction,and A-pattern exotropia’
Though current management include superior rectus(SR) recession alone or combined with posterior tenectomy of superior oblique(SO). However there cannot be a singular dictum for its management as one key cannot fit all locks.

METHODS
Case 1 underwent lateral rectus muscle recession and ‘half-width’ infra-placement with Faden’s operation of SR.
Case 2 underwent a resect-recess procedure with muscle transplantation and posterior tenectomy of SO.
The patients were on regular follow up.

RESULTS
In both the cases, optimum correction is seen in all the 3 components of the syndrome.

CONCLUSION
Optimum outcome acceptable to the patients is seen when surgery is tailored to the patient's magnitude of deviation as seen in both the cases.
