Abstract
A 65 year old male presented with prominence of right eye along with heaviness in upper lid since 6 months.On examination,best corrected visual acuity in right eye was 6/9 and left eye 6/6.There was axial proptosis with 30 degree exotropia and elevation restriction in right eye.Rest anterior and posterior segment revealed no abnormality.MRI Orbit showed right sided bulky Levator palpebral superioris-superior rectus complex,altered signal intensity,fat stranding likely inflammatory pseudotumour.Trial of oral steroids and albendazole was given which showed no improvement.Incisional biopsy was performed suggestive of angiolipoma.Patient was given intravenous methylprednisolone followed by oral steroid.Repeat imaging showed reduced inflammatory changes, however patient had no symptomatic relief. Anterior orbitotomy with mass excision was performed.
Conclusion:Orbital angiolipomas are uncommon and may mimic other orbital lesions.Surgical excision with clear margins offer favourable outcome.
