Abstract

A Case report of 34-year-old male patient who presented with complaints of sudden onset profound loss of vision in right eye since 10 days. He also had evening rise of temperature since 1 month. On examination, he was febrile and best corrected vision was no perception of light in right eye and 6/6 in left eye. Anterior segment examination was normal in both eyes except for 4 mm pupil with relative afferent pupillary defect in right eye. Posterior segment examination showed features of combined central retinal artery and vein occlusion in right eye. Ocular angiography and ultrasonography findings suggested infiltrative etiology. Contrast enhanced MRI Brain and Orbits suggested tuberculoma of optic nerve head and occipital lobe. Chest X-ray showed signs of miliary tuberculosis and CBNAAT was positive. So the final diagnosis of right eye optic nerve head tuberculoma with combined retinal vascular occlusion and pulmonary tuberculosis was made and was started on anti-tubercular treatment.
