Abstract

We present a case of a 45 years old female who presented with left eye diminution of vision since 1 month, BCVA 6/18. Right eye BCVA 6/9. Anterior segment examination of both eyes was unremarkable. The left eye fundus showed a yellowish grey elevated choroidal lesion along the inferotemporal arcade, involving inferior macula. A widefield fundus image was obtained. Fundus autofluorescence showed a hypoautofluorescent lesion with stippled hyperautofluorescence within. OCT showed a choroidal mass with sub retinal fluid and subretinal hyperreflective material (SHRM). OCTA depicted a network at the site of the SHRM with corresponding flow signal, suggestive of an active choroidal neovascular membrane complex. B scan showed a hyper echoic choroidal mass with acoustic back shadowing involving macula, giving a ‘double-optic nerve head appearance’.
Laboratory investigations were done to rule out other differentials and the patient was advised intravitreal anti VEGF injection for the CNVM.
