Abstract
23 year male patient presented with chief complaints of glare, halos and decreased near vision in RE since 1 week. He underwent RE ICL surgery for high myopia 4 weeks ago elsewhere after necessary preop evaluation. At 3 weeks postop he had similar complaints and O/E RE vision was 6/6, N10.IOP 32mm of hg and Pupil was dilated, sluggishly reacting to light. He was started on topical AGMs along with topical steroids and antibiotics. At 4 weeks O/E BCVA in RE 6/6,N10 & LE 6/12, N6. RE pupil was still dilated to 7mm, irregular, sluggishly reacting to light with ICL insitu with vault of 1 corneal thickness.Rest of the findings are WNL.LE both anterior and posterior segment was WNL. We made a diagnosis of RE Urrets Zavalia syndrome post ICL surgery and LE high myopic astigmatism. We advised patient to use pilocarpine 2% eye drops in RE 4 times a day and also suggested about the option of pupilloplasty along with/without ICL explantation.
