Abstract
Purpose:To report a case of uncontrolled hypertension with portal vein thrombosis and multiple comorbidities being diagnosed as Miliary as well as disseminated TB after an ocular examination.
Methods:31/F k/c/o uncontrolled hypertension with acute on chronic kidney disease on hemodialysis presented with abdominal pain. She had a Hb 8 gm% at the time of presentation and on further evaluation was found to have portal vein thrombosis with hepato-splenomegaly, mild ascites and enlargement of retroperitoneal lymph nodes.
Her BCVA was 6/6 in both eyes and on fundus showed infiltrative disc oedema along with background anaemic retinopathy in the presence of choroidal granulomas and tubercles in both eyes. Patient had a h/o positive TB contact.
Result & conclusion :A diagnosis of presumed ocular TB made and was sent for HRCT thorax, which further confirmed the presence of miliary TB. A final diagnosis of disseminated TB was made.
