Abstract
Study Design- To report a rare case and its management.
Purpose-To report an infrequently seen case of adult onset large angle exotopia with paradoxical diplopia surgically treated with muscle transplantation.
Method-A 31/M patient presented with outward deviation of either eye since 15 years managing with CHP. History of troubling diplopia since 2years and worsened since 7 months.
Patient gives history of occasional usage of glasses. He presents with Rt Face turn and facial symmetry maintaining.HCRT shows 40*d exo deviation in the RE and LE central,PBCT confirms the 80PDBI.Diplopia chart confirms uncrossed diplopia which is paradoxical.EOM are full.BE V/A-6/6 and both Ant segment and posterior segments are normal.Patient was treated with True muscle transplantation surgery for RE.
Results-Post op results shows near Orthotropia and within a week diplopia also got resolved.
Conclusion:True Muscle Transplantation Surgery cured the large exotropia along with the paradoxical diplopia.
