Abstract

A 45 years old patient came to Eye OPD complaining of diminution of vision and discharge from both eyes for 1 day. Patient is a chronic alcoholic. The right eye was slightly proptosed. Visual acuity in both eyes was PL+ and PR inaccurate. EOM could not be assessed. Eyelashes in both eyes were matted. Discharge was mucopurulent in nature. Conjunctivae of both eyes showed congestion, corneas were edematous, which was more marked in the right eye than the left eye. IOP in both eyes was elevated. Both eyes showed hypopyon, 1*2.5mm in size in right eye, 1*1mm in the left eye. The anterior chamber in the right eye was not visible; in left eye cells were seen, but other structures could not be seen. Based on the presentation and clinical examination, provisional diagnosis of endogenous endophthalmitis was made. Blood sample has been sent for culture and sensitivity. Currently patient is being managed conservatively with injectable and topical antibiotics, iop lowering agents and mydriatics.
