Abstract
Phacoemulsification surgery is the most gratifying procedure for cataract extraction
We are demonstrating the basics of direct chop method in grade 2 cataract
Injecting Phenocaine intracamerally for topical anaesthesia is followed by routine steps of main wound & side port wound creation
An adequate sized capsulorhexis is key to a succesful outcome
Hydrodissection by injecting small jets of fluid beneath rhexis margin is followed by dialling the nucleus
While doing nucleotomy, embedding the phaco probe in nucleus & bi-fragmenting the nucleus with chopper & probe is nicely demonstrated
Emulsification & aspiration of smaller fragments is done properly without damaging enothelium, posterior capsule or vital intra-ocular structures
Aspiration of epinucleus is done under epinuclear settings
Irrigation & aspiration of cortex is done thoroughly
IOL loading & its smooth movement is tested inside the injector to avoid any entrapment
Insertion of IOL in bag is done
Wounds are hydrated
