Abstract

A 28year old man sustained a date palm thorn injury in his right eye(RE) following bike accident.Vision was PL+,PRaccurate.A concealed nasal scleral rupture(SR) was suspected.Anterior chamber showed pupillary membrane(PM) with complicated cataract.B Scan revealed dense vitreous hemorrhage(VH).We planned to do SR repair with cataract extraction and vitrectomy.PM was removed followed by cataract extraction.As VH was getting cleared, to my surprise,a large nonmetallic foreign body(FB) standing upright embedded in superonasal retina was revealed.A large primary posterior capsulotomy was done and corneal incision was enlarged for FB removal.FB removed was a large date palm thorn which measured 9.5mm.There was an associated retinal detachment.Diligent vitrectomy was done,edges of the tear were trimmed,fluid air exchange was performed patiently to avoid any retinal slippage.360 barrage laser was done.PFCL was removed followed by silicone oil injection.Postoperative vision salvaged to 20/120.
