Abstract

Suprachoroidal haemorrhage(SCH) is a fatal complication of glaucoma filtration surgery. Here we describe its management in a unique way.
A young teen male who is a known case of both eye(BE) primary congenital glaucoma with operated trabeculectomy+ trabeculotomy surgery(at 2 years age) presented with diminished vision in BE i.e. right eye(RE):no light perception, left eye(LE) 6/60; intraocular pressure(IOP) 16mmHg (RE), 42mmHg(LE); BE advanced cupping. He underwent LE re-trabeculectomy surgery. After 6hrs of discharge, he came with severe LE pain and diminished vision .Ultrasound(USG) B scan showed haemorrhagic kissing choroidal detachment(CD). Trans-scleral drainage of SCH with 23G pars plana trocar was done, however serial USGs showed CD recurrence. Trabeculectomy scleral flap resuturing was done. The CD had resolved completely by 3 months with visual acuity 6/24 and IOP 16 mmHg. Recognition of early signs and prompt management of SCH is very essential for rehabilitation of vision.
