Abstract
Case of post keratitis corneal haze, PCIOL subluxated in AC presented to us with blurring of vision after a month of cataract surgery done elsewhere. Had planned for IOL exchange. While operating patient was found to have 3 piece PCIOL in AC but with one haptic broken. Started cutting the IOL in 3 pieces in AC itself through preexisting 2.8mm incision. After rotating the IOL and giving second cut on it, surprisingly found to have second haptic of the IOL broken too. With the help of viscoelastic solution IOL was cut into 3 pieces and removed through 2.8mm. Vitreous in AC removed with the anterior vitrectomy and secondary IOL was safely placed in the sulcus.
Patient was reading 6/12 with the operated eye postoperatively after a week.
One should suspect bilateral broken haptics if finds PCIOL in AC and can be managed through the same mainport Phaco incision of 2.8mm.
