Abstract
Purpose :To deal with the challenges and inadvertent complications during Phaco in complicated uveitic cataract
Surgery .Pt. ( BCVA was PL+, PR inaccurate) complicated cataract with thick papillary membrane.planned for phaco ± PIOL+ pupil expander + IVTA. After corneal ports, synechiolysis and pupil membranectomy done. B-Hex ring used.During phaco ZD noted ,CTR used .IOL got engaged in B-Hex ring and Vitreous prolapsed. AV done after iol removal then other IOL implanted. IVTA given . Air bubble gone during IVTA resulted in IOL displaced , vitreous and iris prolapse. AV done, iris reposited and case closed. At 1 month follow up, IOL was decentred. Reposted in OT for IOL redialing ..
Conclusion :Sometimes Phaco in Uveitic cataracts are more Challenging .This patient had come with PL+, PR inaccurate and other eye was Pthisical. Despite of many challenges and complications during surgery patient get some vision and doing well. So my mentor always said –Every cloud has silver lining.
