Abstract

We present a case of a 29 year old male who experienced gradual, painless & progressive diminution of vision in both eyes over a period of 2 years. Ophthalmological evaluation revealed normal anterior segment findings in both eyes. Visual acuity measurements were recorded as 3/60 in right eye , 6/18(p) in left eye (Snellen’s chart) improving to 4/60 & 6/12(p) with pinhole respectively. Cyclopegic refraction indicated hypermetropia of +11.50diopters in right eye & +5.75diopters in left eye. Best corrected visual acuity was measured as 5/60 (N36) with +6diopters spherical correction in right eye & 6/6 (N6) with +3.75diopters spherical in left eye. Anisometropic amblyopia was present in right eye. Dilated funduscopy revealed bilateral retinal nerve fibre myelination. Systemic examination yielded normal results. This case highlights the clinical presentation of reverse Straatsma syndrome, a rare condition of bilateral retinal nerve fibre myelination associated with significant visual loss.
