Abstract
A 70-year-old man diagnosed with left-eye advanced glaucoma with glaucomatous optic atrophy underwent trabeculectomy with mitomycin-C in the same eye. After surgery, his IOP was 13 mm Hg without any medication. 2 months after the surgery, we restarted bimatoprost 0.03% and ripasudil 0.4% e/d to further reduce the IOP in the left eye, which had increased to 20 mm Hg. On follow-up, he complained of blurred vision that had persisted for the past 15 days; his IOP had decreased to 5 mm Hg. His BCVA was 6/18, and 360-degree Choroidal detachment (CD) was noted. We discontinued bimatoprost & ripasudil e/d and started him on prednisolone e/d 1% 8 times/ day and oral prednisolone, which helped resolve the Choroidal detachment with good IOP control, and his vision improved to 6/9.
